BMJ Case Rep Published online: [please include Day Month Year] doi:10.1136/bcr-2012008273 DESCRIPTION A 9-year-old girl presented with an 8-month history of gelastic seizures and precocious puberty. Clinical evaluation including full neurological assessment was otherwise unremarkable. Endocrine tests were normal and there was no family or surgical history of note. Hypothalamic or tuber cinereum hamartoma is a rare non-neoplastic heterotrophic lesion, which forms part of the spectrum of grey matter heterotopias. It is composed of a disorganised mass of neuronal cells within the tuber cinereum of the hypothalamus. It may occur either as a sessile or as a pedunculated lesion. Presentation occurs in the first decade and manifests with isosexual precocious puberty owing to luteinising hormone releasing hormone secretion, gelastic seizures, hyperactivity, visual deficits and neurodevelopmental delay. Gelastic seizures are characterised by uncontrollable bursts of laughter lasting between 2 and 30 s, and affected children may rarely enter into ‘status gelasticus’. Hypothalamic hamartomas are typically isolated lesions but may also occur in the setting of Pallister-Hall syndrome. They are seen as well-defined round masses on imaging, which measure up to 4 cm in diameter and project inferiorly into the suprasellar or interpeduncular cisterns. The hamartoma is isodense on CT and follows grey matter signal on both T1-weighted (figure 1) and T2-weighted sequences (figure 2). Lack of contrast enhancement is important for radiological diagnosis and distinguishes the tumour from commoner regional pathologies including hypothalamic astrocytomas, germ cell tumours and Langerhans cell histiocytosis. Treatment with luteinising hormone receptor agonists is often effective with surgery being reserved for refractory cases.

Hypothalamic hamartoma associated with a craniopharyngeal canal.

Hypothalamic hamartoma is a rare congenital lesion. We present the case of a 7-year-old girl who suffered from precocious puberty, the cause of which was diagnosed by using MR imaging and CT as pedunculated hypothalamic hamartoma associated with a large craniopharyngeal canal and sellar spine mimicking pituitary duplication.

A case of pure gelastic seizures due to hypothalamic hamartoma with a benign course☆

Hypothalamic hamartoma is a potentially complex entity with diverse clinical manifestations. We report a case of gelastic seizures associated with a hypothalamic hamartoma, which followed a benign course. A 31-year-old woman with episodes of laughter was referred for diagnostic evaluation. Her initial MRI and EEG were reported as normal. However, her episodes of laughter were typical of gelasti...

A Giant Hamartoma of Tuber Cinerium: Conventional MR Imaging and MR Spectroscopy Findings

Hypothalamic hamartoma is a developmental malformation in the region of tuber cinereum and mamillary bodies. It presents the most commonly detectable lesion in patients with precocious puberty and is associated with gelastic seizures. As well as, it can be completely silent to be found only incidentally. Computed tomography (CT) and magnetic rezonans (MR) imaging are preferred imaging modalitie...

Hypothalamic hamartoma presenting with gelastic seizures, generalized convulsions, and ictal psychosis.

We report a case of hypothalamic hamartoma in an adult female who presented with gelastic seizures, generalized convulsions, and ictal aggressive psychotic behavior. Anticonvulsant treatment was ineffective in controlling the epileptic seizures. Surgical excision after accurate imaging diagnosis 3 decades after the onset of symptoms markedly ameliorated her condition. Delayed and erroneous diag...

Psychiatric aspects of patients with hypothalamic hamartoma and epilepsy.

Uncontrolled rage, while long associated with hypothalamic hamartoma, has not been as extensively studied as the epilepsy. Rage can be more detrimental to quality of life than seizures. It is now realized that behavior and aggression improve after a complete resection of the hypothalamic hamartoma correlating with a good seizure control post-surgically. We report on the longitudinal psychiatric...

Central precocious puberty secondary to hypothalamic hamartoma

Introduction Central precocious puberty (CPP) presenting at a very young age is likely to have an underlying pathology. One of the pathologies is hypothalamic hamartoma (HH), a non-neoplastic tumour-like lesion located at the floor of the third ventricle, near the tuber cinereum. Two young children with CPP due to HH without gelastic seizures or mental retardation were successfully managed and ...
